Toolkit
Availability
The Toolkit will have three editions.
1)
The "Big Box" Toolkit, summer 2004
Contains three fully developed training and implementation manuals,
along with videos, posters, CD ROM with database and sample forms,
and other resources.
We were funded to provide Toolkits from our initial print run, what we're
calling the "Big Box" edition (which includes manuals and additional
materials) at no cost to organizations that are members of the asthma/chronic
conditions learning collaboratives or quality improvement networks currently
at work in California. Specifically, these include CAASA, California Asthma
Initiative/Prop 10, California Quality Improvement Collaborative
(non-330 clinics), HRSA Health Disparities Collaborative (Federal 330
clinics in California), and the California Statewide Asthma Collaborative,
and to interested Regional Health Occupations Resource Centers.
Qualifying clinics and organizations will be contacted by your learning
collaborative coordinators. Applicants need to show interest by agreeing
to take a few simple steps detailed in our application/request to ship
form,
and to complete a survey within one month of receipt. Only one Toolkit
per collaborative member is available for free. Subsites may purchase
Toolkits, and collaborative members may purchase additional copies
if they wish.
Organizations that are not members of the networks and collaboratives
listed above may purchase a Toolkit while supplies last (after our grant
commitment to the above organizations have been fulfilled).

about purchases of the "Big Box" will receive response in late
August, 2004.
As supplies are available, you will be sent an application/request to
ship
form, and an order form. Toolkits will be priced on a sliding scale,
according to the type of organization purchasing, from $275 to $700.
Grant funding allows us to offer the lower price to community based clinics
and others, while the higher prices will be charged to for profit and
larger
healthcare organizations. Request order form for details.
The initial "Big Box" edition of the Toolkit is available only
to organizations.
Individuals may purchase manuals from our second run, or may wait
for the commercially published edition.
We are actively looking for a funder to support a national edition of
the
"Big Box" version of the full Toolkit, an edition that would
include all
supplementary resources and materials.
2) The Interim Edition of the Three Toolkit Manuals
and the Spanish Community Health Worker Manual
After
the "Big Box" edition is gone, we will be printing manuals only,
for organizations or individuals. Details will be available on this
website in early fall, 2004.
3) Commercially Published Edition of the Three Toolkit Manuals
We
are looking forward to a national printing of the manuals by a
major commercial publisher, we hope within a year, which will get
thousands of copies into circulation. This edition will facilitate
incorporating chronic conditions management into higher education,
as well as making the manuals readily available nationwide for in
service training and use. Details will be posted to this website as
available.
The Spanish Version of the Community Health Worker Manual
The Community Health Worker Manual has been produced in
Spanish as well as English, due to the immense need for bicultural
bilingual Spanish speaking community health workers in California
and across the country. The Spanish version may be requested,
in fall 2004, by those who have received or purchased a "Big Box"
Toolkit, and who serve a large monolingual Spanish-speaking
population. We will fill these requests on a first come, first served
basis
while supplies last. Pricing for the Spanish version has not yet been
set.
The Spanish version will be available when we offer the interim
edition of the manuals.
